In compliance with the New Jersey Department of Education Guidelines and Regulations, and the Woodcliff Lake Board of Education, any medication which is to be administered during school hours:
- Must have parent or legal guardian complete and sign the Medication Form giving the school nurse permission to administer the medication
- Must have a physician, dentist, or advanced practice nurse complete and sign the order form stating the type of the medication, its dosage, its purpose, and the time to be given, as well as possible side effects.
- All medication must be in its original pharmacy labeled container (ask your pharmacist for a separate bottle for school); Over-the-counter items also must be in their original packaging.
This policy includes ALL medication to be administered in school including over-the-counter items (such as Tylenol, Advil, or any other medication)
The school nurse should be advised of any drug being taken by a child in school or at home, particularly those which might cause a change in behavior.

SELF ADMINISTRATION OF MEDICATION POLICY
Permission may be granted to a student for self-administration of medication for asthma or other potentially life-threatening illness. In addition to a medication order, parent/guardian and physician authorizations, including written certification regarding the student’s condition and capability of self-administration must be provided. ASTHMA ACTION PLAN
Each student authorized to use asthma medication must have an Asthma Action Plan prepared by the student’s physician and submitted to the school nurse.

The administration of epinephrine is allowed by a trained designee in the nurses absence for an anaphylatic reaction.
